1. Complete requirements (see requirements below for specific therapist);

3. ITS, LLC can provide assistance to satisfy IDFPR requirements.
4. ITS, LLC will review submitted requirements, and will email any additional information needed, ITS, LLC will set-up phone and/or in person

Requirements For Foreign-Trained Physical Therapists For The State of Illinois, USA

1. TOEFL (Test of English as a Foreign Language) and TSE (Test of Spoken English) passing score
2. Credentials (diploma, transcript, foreign physical therapy license certificate, etc.)
3. FCCPT Type I Comprehensive Credentials Evaluation (FCCPT Type II Certificate if candidate has obtained ICHP- CGFNS Visa Screen). USCIS recognizes FCCPT Type I OR ICHP-CGFNS for PT Visa Screen.
4. Submit application to:

ILLINOIS DEPARTMENT OF FINANCIAL AND PROFESSIONAL REGULATION (IDFPR)
Attn: Division of Professional Regulation Springfield, ILLINIOS, U.S.A. 62791

5. Candidate will receive Illinois approval through CTS and authority to test from FSBPT
6. Integrated Therapy Specialists, LLC may commence processing of immigrant petition upon receipt of approval.

Requirements For Foreign-Trained Occupational Therapists For The State of Illinois, USA

1. TOEFL (Test of English as a Foreign Language) and TSE (Test of Spoken English) passing score
2. Credentials (diploma, transcript, foreign occupational therapy license certificate, etc.)
3. Visa Credential Verification Certificate for Occupational Therapist (VCVC)
4. Submit application to:

ILLINOIS DEPARTMENT OF FINANCIAL AND PROFESSIONAL REGULATION (IDFPR)
Attn: Division of Professional Regulation Springfield, ILLINOIS, U.S.A 62791

5. National Board for Certification in Occupational Therapy, Inc. (NBCOT) Certificate
6. Integrated Therapy Specialists, LLC will commence processing of immigrant petition upon receipt of the above requirements.
